kjorteo reminded me about that I never got around to finishing up until recently.
After you bring Anghel to Okosan and get him identified as the bird seen by the infirmary, Willexia comes over, looking a bit exasperated, thanking you for finding her wayward student. After explaining where you'd been and what had happened, she nods thoughtfully and then walks off.
She reappears later in Mr. One/Leone's maintenance office, offering to help with your investigation in order to keep an eye on Anghel, who she believes to be instrumental in solving all this. She also states having matters to settle with Dr. Iwamine, though exactly what she doesn't elaborate on at the time.
After the final battle with Hell-Ryouta, Kazuaki pulls the gun he uses to shoot Shuu. Seeing it, Willexia leaps forward to stop him....he responds to her interruption by nonchalantly putting a bullet through her chest before forcibly taking Ryouta into the medical room.
Shuu, somewhat mockingly, asks her why she would do such a thing. Struggling for breath, she explains to him that it was simply her nature to care for others, no matter how monstrous they were...such as him. She then goes on to explain why she initially came to St. Pigeonation's: she had heard rumors of the medical center and the experiments being performed there, rumors she had managed to confirm through a connection she had with the Dove party. It was through that connection that she learned of students like Anghel, and of Operation Hatoful. She transferred to the school in order to uncover the truth of what the Hawk Party was up to here...musing about how ironic it was that, now that she had all the answers and evidence she needed, she'd thrown it all away to save one of the guiltiest culprits in the whole mess.
After Kazuaki is brought back to his senses, he finds that, while fading, Willexia was still alive. He suggests putting her in stasis along with Hiyoko's remains and Ryouta, until such a time that medical technology can find a way to save her.
After everyone else has gone, her spirit speaks to Hiyoko and Ryouta, apologizing for not being able to prevent all of this from happening to them, and stating her intent to make up for it. Hiyoko asks how she could do that with one of them on the brink of death and the other past it...Willexia simply responds by asking them both if they'd ever heard the myth of the phoenix, and, upon their affirmative response, tells them a secret...it is no myth. The one secret she had held back from the world was that she herself was one of the legendary firebirds, watching over the events of this world for ages.
After all this time, however, she was tired, and ready to move on. As such, she knew just how to, asking a pardon for her expression, kill three birds with one stone; she can use her power of reincarnation to, instead of herself, bring Hiyoko back to life and cleanes the Charon virus from Ryouta's body.
As she prepares this, she asks Hiyoko one question...the form she wishes to return in....the answer comes without hesitation.
As Sakuya and the others walk away from the exit to the medical center, they see a brilliant pillar of light and flame shoot up into the heavens. As they stand there wondering what had happened, the exit opens, and out walk a perfectly healthy Ryouta...and a rock dove none of them recognized. The newcomer introduces herself...
...as Hiyoko Tohsaka.
